Preschool Screenings Now Open!
Walk-In Screening Dates & Location:
-- April 1 | 11:00 AM – 6:00 PM
-- April 23 | 3:30 PM – 6:00 PM

New Families: Apply online and then call (815) 802-4954 to schedule a screening.
Returning Families: Call (815) 802-4966 with any questions.

Required Documents for Screening:
• Proof of income
• Birth certificate
• Photo ID
• Medical card (if applicable)
• Proof of residency and two additional proofs of address
